Patient Financial Services

When Paying Your Bill is a Concern

Granite Falls Health has a financial assistance policy. You may view a copy of the Granite Falls Health Financial Assistance Policy here or it is available from Patient Financial Services by calling 1-800-972-6510 and requesting a copy by mail or email. If you have questions or need assistance in completing the application that will be provided to you.

What is Financial Assistance?

Granite Falls Health offers Financial Assistance for emergency or other medically necessary services provided and billed through our Patient Financial Services. This assistance, ranging from a sliding scale reduction up to complete forgiveness of the outstanding balance, is provided to patients who demonstrate financial need.

How to Apply for Assistance

The patient or any person involved in the care of the patient, including a family member or provider, can express financial concerns at any point during the patient’s care. A financial assistance application is available here Charity Care Application or contacting a patient account representative by phone at 1-800-972-6510.

Patients must fully complete the application and include documents not limited to; tax returns, bank statements, pay stubs, property tax. The application process for medical assistance should be followed and any denial information submitted with the application.

What Should I Expect?

Your application will be reviewed and a decision communicated to you. All information is confidential. Eligibility is determined by family size, income (Federal Poverty Guidelines), assets, expenses, employment status and any special consideration the patient and or provider would like to have considered.

Amount Generally Billed

In the case of emergency or other medically necessary care covered under the Financial Assistance Policy, eligible patients may not be charged more than the amounts generally billed to individuals who have insurance covering such care.
